Department of Health Information
The Meadowmount School of Music is required to obtain a permit to operate from the New York State Department of Health. We are required to follow their regulations and are subject to their inspections. Questions regarding our Medical Policy may be directed to the New York State Department of Health, P.O. All federal, state and local health and safety laws will be followed in accorance with COVID regulations. All students, faculty and staff must be fully vaccinated againt COVID and supply proof of vaccination. Prior to arrival all students must have a negative result Molecular PCR Test within 3 days of arrival. Testing well be done on site upon your arrival and then administered weekly as directed. This is being done to ensure the safety of all while attending Meadowmount.
No on-campus or off-campus student will be registered without a fully completed health form including documentation of immunizations if you have been immunized. Withholding or not disclosing accurate medical/health information will result in dismissal with no refund of any fees paid. A physical exam is not required.
In compliance with the New York State Department of Health, prescription and non-prescription medication of any kind may NOT be kept in the dormitories of students under the age of 18. This includes vitamins, tylenol, sleeping aids, topical lotions, cough drops, etc. This policy is strictly enforced and non-compliance will result in dismissal with no refund of any fees paid.
If a parent needs to send medication to a minor, it must be mailed directly to our nurse. Be sure to label the package to the nurse’s attention and enclose the students name in the contents.
More information will be included with the medical forms that will be sent after acceptance into Meadowmount.
